This is a Motorola SM56 modem. Various issues with it were addressed in
3.8.2c2. I noticed that you are running 3.8.2c2, but it looks to me
that the SM56 is running a very old code revision--in fact, the
"Experimental 0.00" response from ati3 looks kind of scary. Go to the
following website, and download the latest firmware for this modem:

There are two versions--one for the ISA version, and one for the PCI.
Make sure you get the right one.
