(PMOD) Modem Help

I cannot find anything on a customer's modem with no box or paperwork. The
"FCC code" he gives me doesn't exist, and all I have is the ATI3 response
of v1.200-k56_DLP. I know is it not a LT or PC-TEL and don't think it is a
USR. Is it a Ambient (New division for Cirrus), Conexant, or possibly a ESS?
