Radius confidence

Sorry about silly question:

WORLD ---- PM2ER ------ Radius Server ------ Internal network
Is safe enough for an ISP to use the above situation, where all the
machines share the same subnet or is necessary to:

WORLD ---- PM2ER ------ Radius Server (one subnet)
|
proxy
|
Internal Network (other subnet)

I know that the second option is harder to set, but the doubt: Do I really
need it ?
