If they are relying solely on RADIUS they have failure states - no
exceptions. The states are part of the protocol definition itself, so it
is unavoidable. Unless they are using an external check (like SNMP) they
have them.

AFAIK it is no. I believe the next-gen server handles them, as well as
a number of other things that have been asked for. But that is a completely
new codebase. AFAIK new feature additions to the current server codebase
are being restrained as it will be obsoleted in the near future.
