Port stuck at USERNAME

Almost every time I do a show sessions on my PM2-E30, port S11 is shown as
status USERNAME.

This just started today, after I flashed my Courier modems, and replaced the
serial cables. Would this be a modem, or a cable problem?

When it happens, my modem won't answer the phone, even though it's set to
auto answer. Unplugging the serial cable, and plugging it back in clears
the problem for a while.
