Session Terminated reason

I upgraded to ComOS 3.3.1c1 and am receiving the
"Session Terminated" reasons, but I could not find
any definitions: what are the possible causes of
each reason?
ie: does Lost Carrier always mean that the call was dropped?

I am looking for definitions in simple terms... to present
some type of statistics to customers (reliability and such).

I looked in the 3.3.1 release notes, but they are
not there yet (probably not updated with 3.3.1c1
info yet).
