The PM is sending an auth request to the RADIUS server, which hears
and replies, but the PM doesn't receive it. After a short time, the
PM resends, assuming the RADIUS server didn't hear it. The RADIUS
server hears it, notices that it's the same as the last one it just
answered, and drops the duplicate, logging the message as it does so.

Possible causes: if your RADIUS server is dual-homed, it might be
replying from the wrong address, causing the PM to think it's being
spoofed and drop the responses, or maybe network noise or some other
network problem is garbling the responses en route to the PM so it
doesn't receive/understand them...
