Me three, but with some useful info.

It hasn't happened to us since we upgraded to 3.3.3 last week, but it
seemed to have happened when the the telco does something to the line
to cause it to reset - a momentary lapse of service.

The PM then doesn't seem to be able to re-establish itself with the
telco properly. It would be nice if the reset command for ISDN ports
went a little farther, perhaps actually trying to reset the hardware
somehow, simulating a power-cycle for that port.
