Once upon a time Brian Kramer shaped the electrons to say...

So you see a request come in, then RADIUS definitely is ACKing, right?
And you see a packet on ptrace coming back?

Odds are it is:
1. Bad config in the PM-3. "port" error can cause problems, if the RADIUS
reply approves one service and the port isn't configured to do it it will
fail. Amongst other things.
2. Bad user config. Maybe the user config has some parameter set that
the PM-3 doesn't like.
3. Combination of the two - ie an interaction between the user settings
and the box config so that neither is really "wrong" just incompatible.
(well, #1 is just a particular case of this general case).

What version of RADIUS server is it?
