USERNAME stuck...

I know that this is discussed in the archives (I've been there...)
but I couldn't find a solution to my exact problem.

I recently upgraded one of our PM's to 3.3.3 and now S0 is good for
one maybe two calls then it gets stuck in USERNAME. I have (of course)
double checked the dip swtiches, yadda yadd... but they all seem fine.
I put the modem on another port and it has been working OK for almost
12hrs so I don't think it could be the modem or cable. DCD is getting
stuck high, but there is no logical reason why it would do so
sporadically on that one port and not others (configured identically BTW).

Any ideas suggestions here are welcome:

Here's what S0 looks like in a "normal" state, unfortunately I don't
have the "dead" state as it is a bit hard reproduce being somewhat
sporadic...
