With reference to telcos that have DMS10 switches:
I was sending messages to this list recently about my (telco's) inability
to get my CT1 to work with my PM3. I was only getting 26.4 and 28.8
connections.
Someone forwarded a spec sheet to me, from one "Kyle" that has
all the magic information. I forwarded it to the telco to no avail,
*UNTIL* it still didn't work and they decided to "really" make
the changes. Then viola, 44-46K every time. I'm happy.

I spoke with someone at the last ISPCON who was having a similar
problem.=A0 As it turned out the TELCO had been somehow running their new
switch in such a way that the incoming calls were being re-routed through
their old analog switch over a CAT5 Cable in the sub-station.=A0 The only
way that the guy found out about it was that he was friends with one of
the techs who allowed him to look through the station.=A0 Basicly the
trouble boiled down to the fact that the TelCo techs were old had and had
little working knowlege of their new digital equipment.
I have also run into a similar problem on the client side when the client
had both their credit card capture machine and computer hooked to a line
that had at one time been running through their AT&T Digital Phone
system. Basicly the point is that the phone system was prventing proper
downstream digital conversion(or the lack there of).
We are on copper her that has had well over 50 years of weather abuse and
we still manage to get connections as high as 44000 when the circuit
distance is close to 10 miles.=A0 Just lucky I guess. :)
