radiusd and duplicate requests

I noticed that I was getting some duplicate records in my radiusd
accounting data, so I started reviewing the code. I found two things:

1. radiusd never checks accounting packets for duplicate IDs. All
accounting packets are logged regardless.

2. The duplicate ID checking for authentication requests is only done
in multi-process mode. If you run in single-process mode, that code is
never executed.

(1) explains my duplicate accounting records. (2) explains why I
sometimes get bogus duplicate login requests.

Has anyone else run into these problems?
