With ISDN MLPPP calls, its the originating side's responsibility to request
the 2nd B channel. Is the Pipeline 130 configured correctly?

No. And you already mentioned the RADIUS port-limit=2.

The PM3 will auto-connect at either 56/64K. Again, this might be a
configuration of the Pipeline 130. If it is configure for data-over-voice?
If it is I think you will only connect at 56K.
