I upgraded several PM-4's to 4.1b15, using the old unix 'pminstall'
utility. Didn't have any problem with any of the boxes that way. I
suspect that PMVision might be the cause here, though I could be wrong.

I wholeheartedly agree here. Cisco's CCO is amazing at being able to find
bugs, their status, potential work-arounds, etc. Something like that
for Lucent would definitely be a big help.
