In an eMachine by any chance? I had a customer report
similar problems with this modem, and an upgrade to the
latest drivers on the eMachine web site seemed to fix
things for him. Unfortunately, your customer seems to
have the latest version already. FYI, the eMachine
web page with the drivers is:

The PCTel drivers are partway down the page, as vR764-9ki.zip.

Also, the 56k=v.Unreliable web page has some init strings
listed for PCTel chipsets. In particular, it recommends
"n0s37=12" to disable v.90. The full web page is here:
