If the user logs out - ie from a shell or requests a disconnect in PPP -
the port immediately resets. The RADIUS accounting packet is sent at that
time. And the modem should hangup when DTR drops as part of the reset.
If the user hangs up (like Trumper does) then the modem should drop DCD and
the port resets immediately upon loss of DCD. (Modem control must be on.)
Rest is same as above.

If the idle timer trips, the port is reset. Same thing happens as if the
user logged out.
