Having done this about 2 years ago I can offer this:
1) Make sure you have proper power (correct plug configs, etc for your UPS's)
2) If you can get smart-jacks installed & tested for T1, POTS etc before
hand that is a major bonus
3) Cell phones, pagers and a "chain of command" for communication to/from
moving people, personnel
4) Assign each employee responsibility for their own workstation, and
associated peripherals.
5) Notify your upstream provider of your move and any alternate phone
numbers they can reach you at (item 3) in case of trouble, or alarms.
6) Plan your move on paper and have several people look at it for omissions
or logistical errors.
7) In regards to item 6, plan your sequence of equipment power down and up,
who will do it and how you will handle the movement of equipment (NOC
related hardware).
8) Hope for the best, plan for the worst
