When you write "latest firmware," please be specific. Is he running version
5.66 from http://www.lucent.com/micro/K56flex/driver2.html ?

Long shot:
Did anything change with the phone line? We have heard of a company
aggressively signing up new customers for local phone service. They're
using a non-Lucent digital loop carrier system that presents a digital
impairment to V.90 that our client code before version 5.57 did not handle
well. The latest 5.66 handles it even better.

Even longer shot:
It's possible that our fix for this problem has some negative effect in
another line condition. We have no evidence of this so far, but we won't
know unless we look for it. To test this theory, visit
http://808news.com/56k/ltwin7.htm to pick up version 5.54. If that works
better than 5.66 on this line, then we need to know. If the customer is
willing, we can work with him to isolate exactly what's happening.
